So do I fill out the form, and then call in a couple days and say I have a duplicate and to combine them? I like reading this and would love to extend my current sub!

Yes, you're correct just wait a few days till the subcription shows up in their system and then give them a call. Like you said, you'll just tell them you have a duplicate subscription and would like to extend it instead.
How do you fill out the form if you are a homemaker and do not have a "Business" name, address, or phone number?

Choose other industries in the 1st drop down box and in the next one choose none. They are both the last options. For the job function choose other function and then choose none again. Finally for the job title choose none too.
For your business name just put your real name and home address.
